Burkitt Lymphoma A type of B-cell lymphoma first brought to wide attention in equatorial Africa by Dennis Burkitt, an Irish surgeon working in that region. In Africa, it usually appears as a facial mass around the jaw, frequently in children. It invariably is associated with the Epstein-Barr virus in the lymphoma cells. An abnormality of chromosome number 8 is also present. Both the chromosome abnormality and viral infection are thought to play a causal role in its onset. In North America, Burkitt lymphoma is far less frequent. This disease usually appears with abdominal masses of lymphoma cells, is not uniformly associated with Epstein-Barr virus, may occur in older individuals, and may involve the marrow and blood. Central Nervous System CNS ; Lymphomas This is the term applied to either the presence of lymphoma only in the brain or spinal cord primary CNS lymphoma ; or the presence of lymphoma in the brain or spinal cord as well as in other locations secondary CNS lymphoma ; . The former or primary type is more often seen as a feature of AIDS lymphoma that follows human immunodeficiency virus infection. The latter or secondary type is usually a feature of aggressive lymphomas such as Burkitt lymphoma. Treatment can include radiation therapy and chemotherapy using drugs and doses that penetrate the central nervous system. Cutaneous T-Cell Lymphoma This subtype of lymphoma principally involves the skin and lymph nodes, or later, other organs. About 3 percent of all lymphomas are of this type. The lymphoma cells have features of T lymphocytes. The disease may wax and wane for many years and may be difficult to diagnose with certainty in its early phases, even with biopsy of the skin. It can be referred to as mycosis fungoides when there is prominent skin involvement. The malignant lymphocytes may enter the blood and, if sufficiently prominent, can mimic some features of chronic lymphocytic leukemia. The lymphocytes that accumulate in the blood have, on close inspection, characteristic folding of their nuclei. This phase of the disease has been referred to as Szary syndrome after the French physician who in 1938 highlighted the unusual and distinctive type of lymphoma cell that entered the blood. Mycosis fungoides and Szary syndrome are now usually referred to as cutaneous T-cell lymphoma. Diffuse Lymphomas This term is used to include the subtypes of lymphoma that have a uniformly disrupted pattern in the lymph node biopsy. When the biopsy specimen is examined under the microscope, the lymphoma cells are spread diffusely throughout the lymph node, in contrast to follicular lymphoma, in which there are clusters or follicles of lymphoma cells. At the end of the powwow it was decided that for L.K. to be cured, a deer must be killed while she watched. I indicated that I did not want to see this, and I do not know if the sacrifice took place, but I was fortunately able to arrange to take L.K. and her mother to New York the following day for studies in our Clinical ResearchCenter. I was worried that far from home, never having seen a river or a bridge, the K. family might experience culture shock. I asked my husband, a psychiatrist, to meet us at the airport. My efforts were for naught; whatever their thoughts, L.K. and her mother showed no signs of shock or surprise. The Zufi are a rather silent people and usually make no fuss at all, but I think that Mrs. K, though she spent most of her time with her hospitalized child, genuinely enjoyed her visit, especially a day at the circus. Now began an exhaustive study of L.K.`s mysterious disease.I first gave her dexamethasone to suppressthe adrenal zona fasciculata, persisting in the hypothesis that L.K. had a 17-hydroxylase defect. However, this only aggravated her hypertension. When I replaced dexamethasone with hydrocortisone, her hypertension worsened. While she was receiving steroid treatment and had very high blood pressure, I lowered the sodium level in her diet. Her blood pressurefell sharply, despite the administration of glucocorticoid. The next step was to try to give her ACTH while she was receiving the low salt diet and normotensive to seewhether her blood pressure would be affected. ACTH administration over 5 days made her blood pressurerise again. After ACTH stimulation ended, we increased the sodium content of her diet, and she became hypertensive again. We then administered chlorothiazide, a diuretic, and her blood pressure fell. Spironolactone, a potassium-sparing diuretic that acts by competitively blocking the mineralocorticoid receptor, brought about remission of her hypertension. We concluded that she had an ACTH-stimulable form of hypertension and that there must be an undetected mineralocorticoid in her blood l-5 ; . We and others attempted to find such a mineralocorticoid in the biological fluids, includ1 and licorice.
