Regular use of bisphosphonate therapy is important to prevent fractures and chronic disability. Indeed, inadequate use of bisphosphonates leads to inadequate reduction of biochemical markers of bone turnover, insufficient increases in BMD and increased fracture risk. In the CANDOO study, a statistically significant difference was observed between consistent and inconsistent bisphosphonate users for increases in lumbar spine BMD after one, two and three years. The consistent users group had significant increases in BMD during each year of the study, whereas there were no significant increases in BMD for the inconsistent user group until year three, when a modest gain of 3.2% was recorded. Many studies have also demonstrated that patients who do not take adequate amounts of bisphosphonate are at greater risk of fracture. In an analysis of a US claims database, it was demonstrated that the likelihood of fracture was reduced if compliance was maintained for one year, compared with patients switching or discontinuing therapy. Adherent patients had a significantly lower risk of fractures at the spine odds ratio OR ; 0.6; p 0.05 ; and hip OR 0.38; p 0.01 ; , compared to non-adherent patients. Similar findings from a Canadian database that included 11, 249 women showed that highly compliant patients MPR 80% ; had a 16% lower fracture rate hazard ratio 0.84; p 0.005 ; than low compliant patients. In the CANDOO study, there was a non-significant trend towards a 27% greater 10-year fracture risk among patients taking therapy inconsistently.

Cholesterol absorption was measured by a fecal dual-isotope ratio method. Mice were dosed i.g. with a mixture of 2 Ci [5, 6- 3H]sitostanol American Radiolabeled Chemicals, Inc., St. Louis, MO ; and 1 Ci [4- 14C]cholesterol NEN Life Science Products, Boston, MA ; . They were then housed individually in fresh cages and stools were collected over the following 3 d. Aliquots of stool and the dosing mixture were extracted, and the ratio of 14C to 3H in each was determined. The percent cholesterol absorption was calculated from these data as described 42 ; . To determine the level of total lipid absorption, the lipid content of the diet and stools was determined gravimetrically. These data, together with the amount of diet consumed and stool excreted both expressed as g d per 100 g bw ; , were used to calculate the fraction of lipid consumed that was absorbed. Fecal bile acid and neutral sterol excretion Stools collected from individually housed mice over 3 d were dried, weighed, and ground to a fine powder. Before I start, I really want to ask something of you. I get to do a lot of these speeches around the United States, mostly in the eight-state region I work in. One of the things that I'm constantly asked by the media is: Are there examples of success stories? So one of the jobs I've taken upon myself is to collect success stories, and what I would like from you are success stories. If you have thoughts about this presentation that you'd like to share with me so that this gets better, I would appreciate that assistance also. I'd like to congratulate Idaho on doing something that I'm not aware any other state has done. The way you've pulled together this media package is the best example of engaging the entire citizenry of the state in becoming more knowledgeable and more involved in finding solutions for Idaho. Idaho has the potential to be a real national leader in this because virtually every challenge that's being played out in the United States is being played out here as well. How you respond to it and how you cross the critical divide, which is to get non-rural people involved, will be an important model to all the rest of the country. In my presentation today, I'd like to start with three caveats. First, there are no easy answers to the important questions we're struggling with. If you hear someone who is trying to promote easy answers, I encourage you to find a video store that has a copy of The Music Man. Remember that Robert Preston movie where the guy came into town and was going to solve all the problems, particularly all the problems related to young people? What the man was really doing was selling trombones and trumpets. Well, there are a lot of music men out there and music women. They're selling easy answers, and a lot of times, it's not the answer they're selling; it's the trombones. So my first caveat is be wary of the music man. My second caveat regards one of the things I see happening in rural America: Rural people are looking for somebody to blame. They're looking for somebody to declare as the enemy. Well, folks, this is not an us-versus-them enterprise. We've already lost the us-them enterprise because rural America is down to about 20% of the population. It's got to be an "all together", a "one-Idaho" solution, not rural Idaho versus the rest of Idaho. Finally, the federal government is not going to solve the problems of Idaho. It's not going to solve the problems of rural Idaho. If problems are to be solved, they will be solved within the state and probably within this room. I spent three years in Washington, D.C., not as much time as the Governor spent there, but I spent three.
