Tropical neurology encompasses a variety of neurologic disorders associated with infectious diseases or nutritional deficiencies. Although most such disorders were initially described in people living in developing countries, neurologists in all parts of the world are increasingly confronted with the diagnostic and therapeutic challenges associated with neurologic disorders in people who have immigrated from or traveled through developing countries. This course will review the diagnosis and management of a variety of "tropical" disorders: parasitic CNS infections, neurocysticercosis, retroviral infections of the nervous system in developing countries and meningoencephalitis and torsemide and ting.

An emergency Caesarean section was performed because of the diagnosis of foetal distress and marginal placental separation. The operation was smooth and a female baby weighing 3.9 kg with Apgar score of eight at one minute and 10 at five minutes was delivered. The lower part of the placenta was found to have separated from the uterine lining, and no obvious retroplacental clot was noted; there was also evidence of pelvic endometriosis. Total blood loss during the operation was approximately 700 ml and the operation lasted for approximately 40 minutes. Postoperative blood pressure was 110 70 mmHg and her pulse rate was approximately 90 beats per minute. The patient was then transferred to the postnatal ward. The formation of a protein in adrenal cells in response to acute ACTH stimulation hasbeen postulated on the basisof a variety of experiments using several inhibitors of protein synthesis 2, 13-16 ; . We have discovered an induced protein with M, 28, 000 and isoelectric point -6.5 in isolated adrenal cells. The protein is not formed in unstimulated cells but is produced in response to acute ACTH Bt2cAMP stimulation or with the same time course and the same ACTH response dose as the increase amount of corticosteroid produced. Further, in cycloheximide added to cells before ACTH prevents stimulation of corticosteroid production and the formation of this protein; this demonstrates the dependence of these processes on protein synthesis. Addition of cycloheximide to ACTHstimulated cells causes the rate of corticosterone production to decrease and the formation of protein i to cease; however, the amountof this protein does not decrease with a tllSof -5 min but remains essentially constant for approximately 15 min. Similar results are obtained fromradiolabel pulse-chase studies, which indicate that, also under stimulated conditions increased corticosteroid production and continuing protein synthesis ; protein i is degraded slowly. Protein i is not produced as a result of the increased levels of corticosteroid subsequenttoACTHaction; this is demonstrated by our studies in which ACTH-stimulated cells were treated with aminoglutethimide, i.e. pregnenolone production by inhibiting the initial step, synthesis. The time course of synthesis of protein i was the same whether or not corticosteroid production was blocked. Nonstimulated cells produced twoproteins p and n ; which have approximately the same molecular weight as protein i but differfrom it in PI.
